Daughters of Grace is a Young Ladies Ministry designed to support and encourage young women ages 11-22  through faith, fellowship, personal development, and life experiences.
This form is required for participation in Daughters of Grace events, outings, and workshops.
If the participant is under 18, this form must be completed by a parent or legal guardian.
